We are continuing to use JVA for insurance purposes for both our beach and indoor programs; thus, please follow the links below to complete & print the 2 required JVA forms--even though some forms are dated with previous years, they are still valid for the the current school year--SIMPLY USE TODAY'S DATES when completing & signing them. (There is no fee for completing the JVA forms--we pay for all player & coach registration fees as a club...) In addition to bringing the 2 completed JVA forms, please complete, print, and bring hardcopies of the 2 Wind n' Sea player forms (links below) to the first day of any clinic, tryout, open gym, etc...
